Never expected the budget vendor to come out behind on TCO. Turns out their process—the one they marketed as "streamlined"—was actually a rebadged workflow from 2019.
The surprise wasn't the price difference. It was how much of it was hidden in fine print I would have missed if I hadn't built the spreadsheet.
A second procurement: different category, same pattern
Around the same time, our general practice wing was upgrading too. We needed three vital signs monitors and two nebulizer machines for the urgent care side. Different category, different vendor pool—same TCO discipline.

The turn: what actually decided it

The outcome

Six-month review: zero unplanned downtime. Imaging throughput is up roughly 12% over our previous clinic's numbers.
The $18,400 overrun from 2023? I found the source. Two years ago, we'd signed a service contract with an auto-renewal clause nobody had flagged. That's a lesson now written into our procurement policy: no signature without a renewal-clause review by two people.
What I'd tell another procurement manager
I can only speak to our situation. We're a mid-size multi-site group with predictable order patterns and centralized IT. If you're a solo practitioner buying one unit, some of this doesn't apply.

- Read the renewal clause. Twice. That 2023 overrun wasn't a product problem. It was a paperwork problem.
- Don't buy "advanced" for its own sake. The question isn't how robotic surgery works or whether AI will change dentistry in ten years. The question is whether the specific tech solves the specific problem in front of you today.
Final number on the new clinic: $94,000 in equipment plus roughly $5,000 projected software cost over five years. Under budget by about $81,000. That goes into our equipment reserve for the next site.
